Texas Crash Report Code Lookup (CR-3)

Most boxes on a Texas Peace Officer's Crash Report (CR-3) hold a number or letter code. Each numbered field points to a numbered list on TxDOT's code sheet, and the same number means different things in different lists. Pick the list, type the code, and see the meaning TxDOT prints for it. The lists are from the code sheet dated 4/1/2023, for crashes on or after April 1, 2023.

This is general information about Texas law, not legal advice, and it does not evaluate your situation.

This tool runs in your browser. What you enter is not sent to InjuryClaimGuide or to any law firm, and it is not saved.

How to read a code

  1. Find the small list number printed beside the field on the report, such as 38 beside the contributing factors boxes.
  2. Pick that list below. A number has no meaning outside its own list: 20 is Driver Inattention in list 38 but Collision With Other Movable Object in list 37.
  3. Type the code to see the entry. Without a list, the lookup shows every list that uses the code.

TxDOT prints the code sheet and its instructions in English only. The Spanish version of this page shows TxDOT's English beside an unofficial Spanish translation.

Type the code as it appears in the box, such as 20, K or FD. What you type stays in your browser.

  • TxDOT's instructions say this section is for the investigating officer to determine which factors or conditions contributed to the crash for each unit, and that "the officer may not have enough evidence to file a traffic charge, but does have enough data to render an opinion as to the contributing factors of the crash" (CR-100, §4.6). The CR-100 heads the section "Investigator's Opinion".
  • List 38 is used twice: "Contributing", up to three factors per unit by priority (CR-100, §4.6.1.2), and "May Have Contributed", up to two factors per unit, "to record the fact that the condition was present, but the investigator is unable to determine whether the factor/condition contributed" (§4.6.1.3).
  • The CR-100 says "Contributing Factor 60 - Unsafe Speed, refers to Under Limit" (§4.6.1.2). Numbers 5 to 13 and 72 are not in list 38; vehicle defects are list 39.

List 16: Injury Severity

CodeTxDOT's official textTxDOT's definition (CR-100, §3.3.29)
ASuspected Serious InjurySevere injury that prevents continuation of normal activities; includes: severe laceration resulting in exposure of underlying tissues/muscle/organs or resulting in significant loss of blood; broken or distorted extremity (arm or leg); crush injuries; suspected skull, chest, or abdominal injury other than bruises or minor lacerations; significant burns (second and third degree burns over 10% or more of the body); unconsciousness when taken from the crash scene; paralysis.
BSuspected Minor InjuryEvident injury such as bruises, abrasions, or minor lacerations which do not incapacitate.
CPossible InjuryInjury claimed, reported, or indicated by behavior but without visible wounds, includes limping or complaint of pain.
KFatal InjuryIf death resulted due to injuries sustained from the crash, at the scene or within 30 days of the crash, the K injury code must be used.
NNot InjuredThe person involved in the crash did not sustain an A, B, C, or K injury.
95AutonomousInjury code 95 is used when Person type is 95 - Autonomous.
99UnknownUnable to determine whether injuries exist. Some examples may include hit and run, fled scene, fail to stop and render aid.
  • The CR-100 says this field captures "the most serious injury for each occupant resulting from the crash", and that every person involved gets a code, including people who were not injured (§3.3.29).

RDRight side damage resulting from sideswipe by meeting or overtaking other vehicle or object.
L&TLeft side and roof damage due to rollover.
R&TRight side and roof damage due to rollover.
  • The code sheet says that "in most cases" the rating is entered as XX-ABC-Y: XX is the direction of force (1-12), ABC the damage description (a 2- or 3-letter code) and Y the damage severity (0-7). The CR-100 says the direction of force is not required, so a rating can also read FD-3 (§3.3.50).
  • The code sheet prints these letters on a vehicle diagram without a key; their meanings are from TxDOT's CR-80 Vehicle Damage Guide, which the CR-100 names as the reference (§3.3.50). For L&T, R&T and TP-0, TxDOT's word for the roof is shown here as "roof".

Read a vehicle damage rating

A damage rating such as 12-FD-3 has three parts: the direction of force, the damage description and the damage severity. Type a rating from the report to see each part.

For example 12-FD-3, FD-3 or VB-1.

A vehicle damaged in more than one area can have two ratings; the CR-80 says the first is the most severely damaged area (§2.3).

Reports on older forms

Reports on crashes from January 1, 2018 to March 31, 2023 use the 1/1/2018 code sheet, and some codes differ (it has no codes for autonomous vehicles, for example). TxDOT posts every form year's code sheet on its crash records forms page.

More about crash reports

A code is the investigating officer's entry on the report. It is not a finding of fault or a court decision, and this tool does not say what any code means for a claim or how an insurer or a court will treat a report. It repeats TxDOT's code lists and is general information, not legal advice. Official sources checked 2026-10-29.
